Medical History and Registration Forms
If you are a new patient referred to Dr. Dean for urologic consultation or if you have previously been referred to Dr. Dean, but have not seen him in more than one year, it is necessary to complete the following set of forms.
Print the forms, complete each section, and then bring the completed forms with you when you come to Dr. Dean's office at the time of Initial Consultation.
Bring your photo ID card and current insurance card with you as well. If you have no insurance please call the office to discuss payment terms.
Having all of these materials prepared before your appointment will save time during the registration process. It will also help you collect your thoughts about your condition. Writing down questions that come to mind before your visit will ensure that you remember to ask them when you're in the office and will enable you to get the most benefit from the time you have with Dr. Dean.
